Payment Plan options:
- Payment Plan Options: Our office offers no interest payment plans for existing patients when the estimated amount due is in excess of $1000 and arrangements have been made in advance with our primary billing specialist and/or our office manager. These arrangements must be made at least 5 business days prior to the scheduled procedure. Approval for payments is not guaranteed in every circumstance.
- If approved, half of the estimated amount due must be paid prior to or on the day of the procedure via debit or credit card which will remain securely in our system for the remainder of your payment plan.
- If approved, the patient or the guarantor will sign a payment agreement detailing the estimated amount due, the initial payment amount and the estimated monthly payments as well as the date each month that the payment will be charged to the card that is securely in our system. The patient / guarantor will receive a signed copy of this agreement prior to leaving our office.
- If approved, the card that is securely in our system will be automatically charged each month. If the amount of your monthly payment changes due to the insurance company’s payment to our office, you will be called with the new amount prior to your card being charged.
- If you close the account of the card that we hold securely on file for your payment agreement or if a payment does not process as per the payment agreement and a new arrangement is not made within the grace period detailed in the payment agreement that you signed, collection procedures and or charges with the local magistrate will begin immediately.
- We accept MasterCard, Visa, Discover and American Express. We do not accept Care Credit unless your card has the MasterCard logo on the card.
Please note: Payment is due at the time of service.
- We accept cash, check and all major credit cards outside of an arranged payment agreement. We do not accept CareCredit unless the card has the MasterCard logo on it.
- As a courtesy to you, we will bill your insurance company and track claims. Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is your responsibility.
